Indications:
TOCIT XR 11 MG is indicated for the prevention of organ rejection in kidney, liver, and heart transplant patients. It may also be used for select autoimmune conditions under physician supervision.
Pharmacology:
Tacrolimus extended-release works as a calcineurin inhibitor that suppresses T-lymphocyte activation by inhibiting calcineurin phosphatase, reducing immune system response responsible for graft rejection. The extended-release formulation maintains steady blood levels over 24 hours.
Dosage:
Dosage is individualized based on transplant type, patient response, and blood tacrolimus levels. Typically started at 0.1-0.2 mg/kg once daily. Blood level monitoring is essential for dose adjustments.
Administration:
Take orally once daily on an empty stomach or consistently with/without food as directed. Do not crush or chew the extended-release tablets.
Interaction:
Interacts with CYP3A4 inhibitors (ketoconazole, erythromycin), inducers (rifampicin, phenytoin), nephrotoxic drugs, and immunosuppressants. Close monitoring and dose adjustments are necessary.
Contraindications:
Hypersensitivity to tacrolimus. Use cautiously in patients with active infections or malignancies.
Side Effects:
Possible side effects include nephrotoxicity, neurotoxicity, hypertension, hyperglycemia, tremors, infections, and gastrointestinal disturbances.
Pregnancy & Lactation:
Use during pregnancy only if benefits outweigh risks. Tacrolimus is present in breast milk; breastfeeding is generally not recommended.
