Indications:
Treatment of hypertension
Management of chronic stable angina pectoris
Treatment of supraventricular tachyarrhythmias including atrial fibrillation and flutter
Pharmacology:
Diltiazem SR is a nondihydropyridine calcium channel blocker that inhibits calcium influx in cardiac and vascular smooth muscles. This causes vasodilation, reduces myocardial oxygen demand, lowers heart rate, and helps control blood pressure.
Dosage:
Typical adult dose: 120 mg once daily. Dose can be adjusted up to 360 mg daily based on clinical response.
Do not crush or chew the sustained-release tablet.
Administration:
Take orally, preferably at the same time each day with or without food. Swallow tablet whole with water.

Beta-blockers
Digoxin
CYP3A4 inhibitors or inducers
Other antihypertensives
Contraindications:
Severe hypotension
Sick sinus syndrome without pacemaker
Second or third-degree AV block
Acute myocardial infarction with pulmonary congestion
Hypersensitivity to Diltiazem
Side Effects:
Common: Headache, dizziness, flushing, peripheral edema, bradycardia, fatigue
Rare: Constipation, rash, nausea, AV block, liver enzyme changes
Pregnancy & Lactation:
Use only if benefit justifies risk. Caution during breastfeeding due to excretion in milk.
Precautions & Warnings:
Regular monitoring of heart rate and blood pressure is recommended. Use cautiously in patients with hepatic or renal impairment and those prone to heart failure.
