Indications:
Dimerol MR 30mg is indicated for the management of type 2 diabetes mellitus, especially in overweight patients, to improve glycemic control when diet and exercise alone are insufficient.
Pharmacology:
Metformin reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
Dosage:
Adults: Typically start with 500 mg twice daily or 850 mg once daily, then adjusted to a maximum of 2000-2550 mg per day based on patient response.
Sustained release formulation is usually taken once daily with the evening meal.
Administration:
Oral administration with meals to reduce gastrointestinal side effects. Do not crush or chew MR tablets.
Interaction:
May interact with iodinated contrast agents, cationic drugs eliminated by renal tubular secretion, and alcohol. Monitor renal function regularly.
Contraindications:
Renal impairment, metabolic acidosis, diabetic ketoacidosis
Hypersensitivity to Metformin
Conditions predisposing to hypoxia (e.g., severe cardiac or respiratory insufficiency)
Side Effects:
Common side effects include gastrointestinal discomfort, nausea, diarrhea, and rarely lactic acidosis, which is a medical emergency.
Pregnancy & Lactation:
Use only if clearly needed and prescribed. Consult doctor before use during pregnancy and lactation.
Precautions & Warnings:
Regular monitoring of renal function is essential.
Avoid alcohol consumption.
Discontinue prior to radiologic studies involving iodinated contrast.
