Indications:

OLMETIC is primarily indicated for:

  • Hypertension (High Blood Pressure): To lower blood pressure in adults and children aged 6 years and older.

  • Chronic Kidney Disease (in some cases): It may be used as part of a comprehensive treatment strategy for patients with kidney disease related to hypertension, as it helps protect kidney function by reducing blood pressure.

Pharmacology:

OLMETIC contains Olmesartan Medoxomil, which is an angiotensin II receptor blocker (ARB). Angiotensin II is a hormone that causes blood vessels to constrict, raising blood pressure. By blocking the action of this hormone, OLMETIC relaxes blood vessels, helping them dilate and thereby reducing blood pressure.

Olmesartan works by selectively inhibiting the angiotensin II receptor type 1 (AT1), which results in the vasodilation (relaxation of blood vessels) and a reduction in the secretion of aldosterone, a hormone involved in sodium and water retention. This leads to a reduction in blood pressure.

Dosage:

  • For Adults with Hypertension: The usual starting dose is 20 mg once daily, which may be increased to 40 mg once daily if necessary, depending on the patient's response and medical condition.

  • For Children (6 years and older): The typical starting dose is 10 mg once daily, with a possible increase to 20 mg after 2 weeks, based on the child’s weight and blood pressure response.

Note: Dosage may be adjusted by the healthcare provider based on the individual patient’s needs.

Administration:

  • Oral use: OLMETIC should be taken orally, once a day, with or without food.

  • Swallow the tablet whole with a glass of water.

  • Take the medicine at the same time each day to help remember.

  • Do not crush or chew the tablet.

Interaction:

  • Diuretics: When used with diuretics (especially those that cause potassium loss), there may be an increased risk of low blood pressure (hypotension), especially after the first dose of OLMETIC.

  • Potassium Supplements: Caution should be taken when using potassium supplements or potassium-sparing diuretics as OLMETIC may increase potassium levels.

  • Non-Steroidal Anti-Inflammatory Drugs (NSAIDs): Prolonged use of NSAIDs may reduce the effectiveness of OLMETIC and increase the risk of kidney problems.

  • Lithium: OLMETIC may increase the levels of lithium in the blood, requiring more frequent monitoring.

Always inform your doctor about any other medications you are taking, including over-the-counter drugs, herbal supplements, or vitamins.

Contraindications:

  • Hypersensitivity: Do not use OLMETIC if you are allergic to Olmesartan or any component of the medication.

  • Pregnancy: OLMETIC is contraindicated during pregnancy, especially during the second and third trimesters, as it can harm the fetus.

  • Severe Liver or Kidney Disease: OLMETIC should not be used in patients with severe liver or kidney dysfunction.

  • Angioedema: If you have a history of angioedema (swelling of the deeper layers of the skin) related to the use of an ACE inhibitor or ARB, you should not use OLMETIC.

Side Effects:

  • Common side effects: Dizziness, headache, fatigue, and nausea.

  • Serious side effects: Low blood pressure (hypotension), elevated potassium levels (hyperkalemia), kidney dysfunction, angioedema (swelling of the face, lips, throat), and fainting.

  • Less common side effects: Back pain, abdominal pain, and allergic reactions.

If you experience any of the following serious side effects, seek medical attention immediately: difficulty breathing, swelling of the face, lips, tongue, or throat, irregular heartbeat, or severe dizziness.

Pregnancy & Lactation:

  • Pregnancy: OLMETIC is classified as Category D in pregnancy. It should not be used during pregnancy, particularly in the second and third trimesters, due to the risk of harm to the unborn baby.

  • Lactation: It is not known whether Olmesartan passes into breast milk. It is generally recommended that OLMETIC should not be used during breastfeeding, especially because it could affect the baby’s kidney function.
