Indications:
PEFLOX 400 MG is indicated for the treatment of various bacterial infections, including:
Gastrointestinal Infections: Enteric fever, acute bacterial diarrhea, and chronic Salmonella carriers.
Urinary Tract Infections: Cystitis, pyelonephritis, prostatitis, epididymitis, and uncomplicated and complicated urethritis.
Respiratory Tract Infections: Pneumonia, acute and chronic bronchitis, cystic fibrosis, bronchiectasis, and empyema.
Ear, Nose, and Throat Infections: Otitis media, otitis externa, sinusitis, and tonsillitis.
Bone and Joint Infections: Osteomyelitis and arthritis.
Skin and Soft Tissue Infections: Infected ulcers, wound infections, abscesses, cellulitis, erysipelas, and infected burns.
Pelvic Infections: Salpingitis, endometritis, and pelviperitonitis.
Abdominal and Hepatobiliary Infections: Peritonitis, intra-abdominal abscess, cholangitis, cholecystitis, and empyema of the gall bladder.
Septicemia, Bacteremia, Gonorrhea, Endocarditis, and Meningitis.
Pharmacology:
Pefloxacin is a synthetic antibacterial agent belonging to the fluoroquinolone group. It exerts its action by inhibiting the subunit-A of DNA gyrase (Topoisomerase II), leading to bactericidal effects and a broad spectrum of activity.
Dosage:
Adults with normal hepatic function: 400 mg tablet every 12 hours (morning and evening) with meals to avoid gastrointestinal disturbances. An initial loading dose of 800 mg may be given to achieve effective blood concentration more rapidly.
Adults with hepatic insufficiency: In patients with severe hepatic insufficiency or reduced blood supply to the liver, the daily doses should be readjusted by increasing the intervals between doses. Dosage need not be reduced in renal insufficiency.
Duration of Treatment:
Usually 5 to 10 days.
For chronic prostatitis, cystic fibrosis, and chronic Salmonella carriers: 14 days.
For endocarditis: Up to 6 months.
For chronic osteomyelitis: Up to 1 year.
For gonorrhea: 2 tablets as a single dose.
Administration:
Oral administration; should be taken with meals.
Interaction:
When taken concomitantly with antacids containing aluminum and/or magnesium hydroxide gel, absorption of Pefloxacin is reduced. Therefore, concurrent administration of these agents with Pefloxacin should be avoided.
Contraindications:
Pefloxacin is contraindicated in patients with known hypersensitivity to quinolones, children under 15 years of age, pregnant and lactating mothers, patients with inborn glucose-6-phosphate dehydrogenase deficiency, and patients with a history of tendon lesions, tendonitis, or tendon rupture.
Side Effects:
Occasionally observed reactions include:
Gastrointestinal disturbances: Epigastric pain, nausea, vomiting, diarrhea, dyspepsia.
Central nervous system disturbances: Headache, insomnia.
Hypersensitivity reactions: Skin rash, pruritus, photosensitization.
Others: Myalgia and/or arthralgia, tendonitis, rupture of Achilles tendon.
Pregnancy & Lactation:
Pefloxacin has been assigned to pregnancy category C by the FDA. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing woman.

Keep below 30°C temperature, away from light and moisture. Keep out of the reach of children.
