Hypertension (উচ্চ রক্তচাপ)

Pharmacology:

DISYS PLUS 80/12.5 Tablet is a combination of two antihypertensive agents: Valsartan, an angiotensin II receptor blocker (ARB), and Hydrochlorothiazide, a thiazide diuretic. Valsartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure. Hydrochlorothiazide increases sodium and water excretion, reducing blood volume and further lowering blood pressure.

Dosage & Administration:

Hypertension: One tablet once daily. Dosage may be adjusted based on patient response.
Can be taken with or without food.
Maximum recommended dose: Valsartan 320 mg and Hydrochlorothiazide 25 mg per day.

Interaction:

Caution with potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium due to risk of hyperkalemia.
NSAIDs may reduce the antihypertensive effect and increase the risk of renal impairment.
Concurrent use with alcohol, barbiturates, or narcotics may potentiate antihypertensive effect or cause orthostatic hypotension.
Antidiabetic drugs (oral agents and insulin): dosage adjustment may be required.
Cholestyramine and colestipol resins may impair absorption of Hydrochlorothiazide.

Contraindications:

Hypersensitivity to Valsartan, Hydrochlorothiazide, or any component of the formulation.
Anuria.
Hypersensitivity to other sulfonamide-derived drugs.
Severe hepatic impairment, biliary cirrhosis, or cholestasis.
Pregnancy, especially in the second and third trimesters.

Side Effects:

Common: Headache, dizziness, fatigue, abdominal pain, cough, diarrhea, nausea.
Rare: Hyperkalemia, hypotension, renal impairment, rash, muscle cramps, back pain, insomnia, vertigo.

Pregnancy & Lactation:

Pregnancy: Contraindicated due to risk of fetal injury or death.
Lactation: Not recommended; Hydrochlorothiazide is excreted in breast milk.

Precautions & Warnings:

Monitor renal function and serum potassium levels, especially in patients with renal impairment.
Use caution in patients with heart failure or post-myocardial infarction.
Not recommended for pediatric use; safety and efficacy not established.
