Indications:
SERGEL 40 MG Capsule is indicated for:

  • Management of Gastroesophageal Reflux Disease (GERD).

  • Healing and prevention of peptic ulcers, especially those caused by H. pylori or NSAID use.

  • Zollinger-Ellison syndrome, characterized by excessive stomach acid production.

  • Erosive esophagitis.

Pharmacology:
Esomeprazole, the active ingredient, is a proton pump inhibitor (PPI) that inhibits the hydrogen-potassium ATPase enzyme in gastric parietal cells. This action significantly reduces stomach acid production, alleviating symptoms and promoting mucosal healing.

Dosage and Administration:

  • Adults: 40 mg once daily, preferably at least 1 hour before meals.

  • The duration of treatment varies based on the condition, usually 4-8 weeks for GERD.

  • Swallow the capsule whole; do not crush or chew.

Interaction:

  • May reduce the absorption of medications like ketoconazole or atazanavir that require an acidic gastric pH.

  • Use with caution in patients taking anticoagulants like warfarin due to an increased risk of bleeding.

  • Concomitant use with clopidogrel may reduce its effectiveness.

Contraindications:

  • Hypersensitivity to esomeprazole, other proton pump inhibitors, or any excipients in the formulation.

  • Severe liver impairment without appropriate dose adjustments.

Side Effects:

  • Common: Headache, nausea, abdominal discomfort, diarrhea, and flatulence.

  • Rare but serious: Hypomagnesemia, vitamin B12 deficiency, and increased risk of fractures with prolonged use.

Pregnancy & Lactation:

  • Pregnancy: Esomeprazole is classified as FDA Category B; safe when prescribed by a healthcare provider.

  • Lactation: Caution is advised as esomeprazole may pass into breast milk.

Precautions & Warnings:

  • Long-term use may lead to increased risks of gastrointestinal infections like Clostridium difficile.

  • Monitor magnesium and vitamin B12 levels during extended treatment.

  • Discontinue use immediately if severe hypersensitivity reactions occur.
