Indications:
NIRVANA 5 MG is indicated for:
Major Depressive Disorder (MDD)
Generalized Anxiety Disorder (GAD)
Panic Disorder (low-dose initiation)
Social Anxiety Disorder (off-label)
Pharmacology:
Escitalopram, a selective serotonin reuptake inhibitor (SSRI), increases the level of serotonin in the brain. It works by blocking the reabsorption of serotonin in the brain, thereby improving mood, emotional balance, and reducing anxiety symptoms.
Dosage & Administration:
Initial dose: 5 mg once daily
May be increased based on patient response (usually up to 10–20 mg/day)
Should be taken with or without food
Best taken at the same time each day
Elderly or liver-impaired patients may require lower doses
Interaction:
Risk of serotonin syndrome with MAOIs, triptans, or other serotonergic agents
May enhance effects of alcohol and sedatives
Increased bleeding risk with NSAIDs or anticoagulants
QT prolongation risk with certain antiarrhythmics
Contraindications:
Hypersensitivity to Escitalopram
Concurrent use with MAOIs or linezolid
Uncorrected electrolyte imbalance (e.g. hypokalemia) in patients at risk for QT prolongation
Side Effects:
Common: Nausea, drowsiness, insomnia, dizziness, dry mouth
Less common: Fatigue, sexual dysfunction, sweating, restlessness
Rare: Suicidal thoughts, serotonin syndrome, hyponatremia, seizures
Pregnancy & Lactation:
Should be used during pregnancy only if clearly necessary
May cause neonatal withdrawal symptoms
Caution during lactation as it is secreted in breast milk
Precautions & Warnings:
Close monitoring is advised during initial weeks of therapy
Avoid abrupt discontinuation to prevent withdrawal symptoms
Caution in patients with a history of mania or seizures
Avoid alcohol and activities requiring alertness initially
