Indications:
EVAGLIP 25 MG/5 MG is indicated for:

  • Type 2 diabetes mellitus in adults who are not adequately controlled on monotherapy with vildagliptin or metformin.

  • As an adjunct to diet and exercise to improve glycemic control.

Pharmacology:
This medication combines:

  • Vildagliptin 50 mg daily dose equivalent (5 mg twice daily) – a DPP-4 inhibitor that enhances incretin hormone activity, stimulating insulin release and reducing glucagon secretion.

  • Metformin 25 mg – a biguanide that reduces hepatic glucose production and improves peripheral insulin sensitivity. (Note: Low-dose formulation for patients requiring mild metformin exposure)

Dosage & Administration:

  • Recommended dose: One tablet twice daily with meals.

  • Adjustments may be made by the physician based on individual response and renal function.

  • Always administer with food to minimize GI side effects.

Interaction:

  • Use caution with other antidiabetic drugs (e.g., sulfonylureas, insulin) – may increase hypoglycemia risk.

  • Alcohol may increase risk of lactic acidosis with metformin.

  • Possible interaction with diuretics, corticosteroids, and beta-blockers.

Contraindications:

  • Hypersensitivity to vildagliptin, metformin, or formulation components

  • Renal impairment (eGFR <30 mL/min)

  • Metabolic acidosis or lactic acidosis

  • Hepatic dysfunction or liver enzyme elevations

Side Effects:

  • Common: Diarrhea, nausea, flatulence, headache

  • Less common: Abdominal discomfort, dizziness

  • Rare: Lactic acidosis, pancreatitis, hepatic dysfunction, rash

Pregnancy & Lactation:
Not recommended unless clearly necessary. Metformin may pass into breast milk; vildagliptin's effects on breastfeeding are unknown. Use with caution.

Precautions & Warnings:

  • Regularly monitor renal and hepatic function

  • Discontinue temporarily in conditions such as dehydration, severe infection, or surgery

  • Monitor for symptoms of lactic acidosis (e.g., muscle pain, breathing difficulty)

  • Avoid alcohol

Therapeutic Class:
Oral antidiabetic combination – DPP-4 inhibitor + Biguanide

Storage Conditions:
Store below 30°C, protected from light and moisture. Keep out of reach of children.
