Indications:
SERGEL 40 MG IV Injection is indicated for:
Management of Gastroesophageal Reflux Disease (GERD) when oral therapy is not feasible.
Treatment of peptic ulcers and erosive esophagitis.
Zollinger-Ellison syndrome in critical care settings.
Prevention of stress-induced ulcers in hospitalized patients.
Pharmacology:
Esomeprazole is a proton pump inhibitor (PPI) that blocks the H+/K+ ATPase enzyme system in gastric parietal cells. This action effectively suppresses stomach acid secretion, providing relief from symptoms and facilitating healing in acid-related conditions. The IV form ensures rapid and effective acid suppression in hospitalized or critically ill patients.
Dosage and Administration:
Adults: 40 mg once daily, administered intravenously over 3 minutes or as an infusion over 10-30 minutes.
For severe cases, repeat dosing may be required as advised by a healthcare provider.
Preparation and Administration:
Reconstitute the vial with the provided solvent or as per manufacturer guidelines.
Administer immediately after preparation to avoid degradation.
Interaction:
May reduce the efficacy of drugs requiring acidic pH for absorption (e.g., ketoconazole, atazanavir).
Use with caution in patients on anticoagulants like warfarin due to an increased risk of bleeding.
May reduce the effectiveness of clopidogrel when used concurrently.

Severe hepatic impairment without dose adjustment.
Side Effects:
Common: Headache, injection site reactions, nausea, diarrhea.
Rare: Hypomagnesemia, bone fractures with long-term use, and severe allergic reactions.
Pregnancy & Lactation:
Pregnancy: Esomeprazole is categorized as FDA Category B; safe when prescribed by a healthcare professional.
Lactation: Use with caution; esomeprazole may pass into breast milk.
Precautions & Warnings:
Avoid prolonged use without periodic assessment to prevent potential adverse effects like vitamin B12 deficiency.
Monitor magnesium levels during prolonged therapy.
Discontinue use and consult a physician if severe hypersensitivity reactions occur.
Therapeutic Class:
Proton Pump Inhibitor (PPI)
Storage Conditions:
