Indications:
ORICEF 1 GM is indicated for the treatment of:

  • Lower respiratory tract infections

  • Urinary tract infections (UTIs)

  • Skin and soft tissue infections

  • Intra-abdominal infections

  • Bone and joint infections

  • Bacterial septicemia

  • Meningitis

  • Gonorrhea

  • Surgical prophylaxis

Pharmacology:
Ceftriaxone is a third-generation cephalosporin antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). It is highly stable against most beta-lactamases and effective against a wide range of Gram-positive and Gram-negative organisms.

Dosage & Administration:

  • Adults: 1–2 g once daily or in two divided doses

  • Severe infections: Up to 4 g/day

  • Children: 50–75 mg/kg/day

  • Administer via intravenous (IV) or intramuscular (IM) injection

  • Reconstitute with appropriate diluent before use

Interaction:

  • May increase nephrotoxicity when used with aminoglycosides

  • May increase bleeding risk with anticoagulants

  • Calcium-containing IV solutions must not be co-administered in neonates

Contraindications:

  • Hypersensitivity to cephalosporins

  • Known allergy to penicillins (cross-reactivity risk)

  • Hyperbilirubinemic neonates (risk of kernicterus)

Side Effects:

  • Common: Diarrhea, rash, pain at injection site

  • Less common: Eosinophilia, elevated liver enzymes

  • Rare: Anaphylaxis, pseudomembranous colitis, hemolytic anemia

Pregnancy & Lactation:

  • Pregnancy Category B: Generally considered safe

  • Excreted in breast milk; use with caution in lactating mothers

Precautions & Warnings:

  • Monitor renal and hepatic function during prolonged therapy

  • Discontinue if signs of hypersensitivity occur

  • Caution in patients with history of gastrointestinal disease, especially colitis

  • Ensure adequate hydration during high-dose therapy
