Indications:
OPAL 20 MG is indicated for the treatment of:
Gastroesophageal Reflux Disease (GERD)
Duodenal and Gastric Ulcers
Zollinger-Ellison Syndrome
Erosive Esophagitis
H. pylori eradication (as part of combination therapy)
Pharmacology:
Omeprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by inhibiting the H+/K+ ATPase enzyme system in the gastric parietal cells. It reduces both basal and stimulated acid production, promoting ulcer healing and symptom relief.
Dosage & Administration:
Usual adult dose: 20 mg once daily before a meal
Duration: 4–8 weeks depending on condition
In H. pylori eradication: Used with antibiotics (e.g., amoxicillin and clarithromycin)
Dose adjustment needed in hepatic impairment
Interaction:
May decrease absorption of drugs like ketoconazole, atazanavir
May increase blood levels of warfarin, phenytoin, diazepam
Caution with clopidogrel as it may reduce antiplatelet effect
Contraindications:
Known hypersensitivity to Omeprazole or other PPIs
Severe liver disease (use cautiously)
Concomitant use with nelfinavir
Side Effects:
Common: Headache, abdominal pain, nausea, constipation, flatulence
Less common: Dizziness, rash, dry mouth
Long-term use: Risk of vitamin B12 deficiency, hypomagnesemia, bone fracture
Pregnancy & Lactation:
Generally considered safe in pregnancy (Category C)
Excreted in breast milk; use with caution during lactation
Precautions & Warnings:
Rule out gastric malignancy before starting therapy
Long-term use may cause nutrient absorption issues
Caution in patients with osteoporosis
May mask symptoms of serious gastrointestinal diseases
