Indications:
SERGEL 20 MG Capsule is indicated for:
Gastroesophageal Reflux Disease (GERD).
Peptic ulcers associated with H. pylori infection or NSAID use.
Zollinger-Ellison syndrome.
Erosive esophagitis.
Long-term management of acid-related disorders.
Pharmacology:
Esomeprazole is a proton pump inhibitor (PPI) that blocks the hydrogen-potassium ATPase enzyme in the gastric parietal cells. This action reduces gastric acid secretion, providing relief from acid-related symptoms and promoting healing of ulcers and erosions.
Dosage and Administration:
Adults: 20 mg once daily, at least 1 hour before meals.
Duration of therapy varies based on the condition (e.g., 4-8 weeks for GERD).
Swallow the capsule whole; do not crush or chew.
Interaction:
May reduce the absorption of drugs like ketoconazole and atazanavir that require acidic conditions for absorption.
Concurrent use with warfarin may increase bleeding risk; monitor INR levels.
Omeprazole may interfere with the effectiveness of clopidogrel.
Contraindications:
Hypersensitivity to esomeprazole, other PPIs, or any component of the formulation.
Severe liver impairment without dose adjustment.
Side Effects:
Common: Headache, diarrhea, nausea, flatulence, abdominal pain.
Rare but serious: Vitamin B12 deficiency with prolonged use, bone fracture risk, and severe allergic reactions.
Pregnancy & Lactation:
Pregnancy: Esomeprazole is classified as Category B by the FDA. Safe when prescribed by a doctor.
Lactation: Use with caution as it may pass into breast milk.
Precautions & Warnings:
Prolonged use may increase the risk of Clostridium difficile infection or bone fractures.
Monitor magnesium levels during prolonged therapy.
Discontinue immediately if hypersensitivity reactions occur.
