Indications:
NIRVANA 20 MG is indicated for the treatment of:

Major Depressive Disorder (MDD)
-
Generalized Anxiety Disorder (GAD)
-
Panic Disorder
-
Social Anxiety Disorder (off-label use)
-
Obsessive-Compulsive Disorder (off-label use)
Pharmacology:
Escitalopram is a selective serotonin reuptake inhibitor (SSRI). It enhances serotonergic activity by inhibiting the reuptake of serotonin (5-HT) into the presynaptic neuron. This increase in serotonin levels helps elevate mood and reduce anxiety.
Dosage & Administration:

Usual adult dose: Start with 10 mg once daily, may increase to 20 mg/day after 1 week depending on patient response

Should be taken once daily, at the same time each day
-
Can be taken with or without food

Dose adjustments may be necessary in elderly patients or those with hepatic impairment
Interaction:
-
Avoid concomitant use with MAOIs or other serotonergic agents to prevent serotonin syndrome
-
Increased bleeding risk with NSAIDs, aspirin, or anticoagulants
-
Alcohol may enhance sedative effects
-
Caution with drugs that prolong QT interval
Contraindications:
-
Hypersensitivity to Escitalopram
-
Concomitant use with MAO inhibitors (within 14 days)
-
Uncorrected electrolyte disturbances (e.g., hypokalemia, hypomagnesemia) in patients at risk for QT prolongation
Side Effects:
-
Common: Nausea, insomnia, drowsiness, dizziness, dry mouth

Less common: Weight changes, sexual dysfunction, agitation

Serious: Suicidal ideation, serotonin syndrome, hyponatremia, QT prolongation
Pregnancy & Lactation:
-
Use during pregnancy only if clearly needed and prescribed
-
May cause withdrawal symptoms in neonates if taken late in pregnancy
-
Caution advised during breastfeeding as the drug is excreted in breast milk
Precautions & Warnings:
-
Monitor closely for worsening depression or suicidal thoughts
-
Gradual dose reduction is recommended to avoid withdrawal symptoms
-
Use caution in patients with seizure disorders or liver impairment
-
May impair judgment—avoid driving or operating machinery initially
