Indications:

Replet 75 mg Tablet is indicated for:

  • Acute Coronary Syndrome (ACS): Reduces the rate of myocardial infarction (MI) and stroke in patients with non-ST-segment elevation ACS (unstable angina/NSTEMI).

  • Recent MI, Recent Stroke, or Established Peripheral Arterial Disease: Reduces the rate of MI and stroke in patients with established peripheral arterial disease or with a history of recent MI or stroke.

Pharmacology:

Clopidogrel is a prodrug that inhibits platelet activation and aggregation through the irreversible binding of its active metabolite to the P2Y12 class of ADP receptors on platelets. Dose-dependent inhibition of platelet aggregation can be seen 2 hours after single oral doses. Repeated doses of 75 mg per day inhibit ADP-induced platelet aggregation on the first day, and inhibition reaches steady state between Day 3 and Day 7. 

Dosage and Administration:

  • Acute Coronary Syndrome: Initiate with a single 300 mg (4 tablets) oral loading dose, then continue at 75 mg once daily.

  • Recent MI, Recent Stroke, or Established Peripheral Arterial Disease: 75 mg once daily orally without a loading dose.

  • Can be taken with or without food.

Interaction:

  • Increased risk of bleeding when used with NSAIDs, warfarin, SSRIs, or SNRIs.

  • Avoid concomitant use with omeprazole or esomeprazole as they inhibit CYP2C19, affecting Clopidogrel metabolism.

  • Avoid concomitant use with repaglinide due to increased plasma concentrations of repaglinide. 

Contraindications:

  • Hypersensitivity to Clopidogrel or any component of the product.

  • Active pathological bleeding such as peptic ulcer or intracranial hemorrhage.

Side Effects:

  • Common: Bleeding, diarrhea, gastrointestinal discomfort, skin reactions.

  • Rare: Acquired hemophilia, anemia, angioedema, arthralgia, arthritis, bone marrow disorders. 

Pregnancy & Lactation:

  • There are no adequate and well-controlled studies in pregnant women. Replet 75 mg should be used during pregnancy only if clearly needed. It is unknown whether Clopidogrel is excreted in human breast milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother. 

Precautions & Warnings:

  • Clopidogrel is a prodrug; metabolism to its active metabolite is impaired by genetic variations in CYP2C19 and by drugs that inhibit CYP2C19 such as omeprazole and esomeprazole.

  • As it inhibits platelet aggregation for the lifetime of the platelet (7-10 days), the risk of bleeding may increase.

  • Discontinue 5 days prior to elective surgery that has a major risk of bleeding.

  • Thrombotic Thrombocytopenic Purpura (TTP) has been reported and requires urgent treatment including plasmapheresis. 

Therapeutic Class:

  • Antiplatelet drugs.
