Indications:
NITRIN SR 6.4 MG is indicated for:
Prevention of angina pectoris associated with coronary artery disease
Management of chronic stable angina
Sometimes used as adjunctive therapy in heart failure (off-label)
Pharmacology:
Glyceryl Trinitrate (GTN) is a nitrate that works by relaxing vascular smooth muscle, leading to vasodilation. It reduces myocardial oxygen demand by decreasing preload and afterload, and improves oxygen delivery to the heart, reducing chest pain.
Dosage & Administration:
Usual adult dose: 6.4 mg once or twice daily, adjusted as per response
Administer with water, preferably at the same time daily
Ensure a nitrate-free interval (typically at night) to avoid tolerance
Interaction:
Serious hypotension can occur with phosphodiesterase inhibitors (e.g., sildenafil, tadalafil)
Potentiates the effect of antihypertensives and alcohol
Use cautiously with diuretics, calcium channel blockers, and beta-blockers
Contraindications:
Known hypersensitivity to nitrates
Severe anemia
Hypotension or circulatory collapse
Concomitant use with PDE-5 inhibitors
Side Effects:
Common: Headache, dizziness, lightheadedness, flushing
Less common: Nausea, tachycardia, hypotension
Rare: Methemoglobinemia, skin rash
Pregnancy & Lactation:
Use during pregnancy only if clearly necessary and prescribed
Safety during lactation is not well-established; use with caution
Precautions & Warnings:
Do not use continuously without medical supervision to avoid tolerance
Avoid sudden posture changes to prevent orthostatic hypotension
Monitor blood pressure regularly in long-term use
Use with caution in patients with glaucoma or increased intracranial pressure
