Indications:
SILINOR 50 MG Tablet is indicated for:
Treatment of major depressive disorder.
Management of neuropathic pain.
Adjunct therapy for anxiety and insomnia in some cases.
Pharmacology:
Nortriptyline is a tricyclic antidepressant (TCA) that inhibits the reuptake of norepinephrine and serotonin, increasing their availability in the central nervous system. This action helps alleviate symptoms of depression and provides relief from chronic neuropathic pain.
Dosage and Administration:
Adults:
Depression: 25-50 mg once daily or in divided doses; maximum 150 mg/day.
Neuropathic pain: Start with 10-25 mg at bedtime, gradually increasing as needed.
Take with or without food, preferably at the same time each day.
Interaction:
Avoid concurrent use with monoamine oxidase inhibitors (MAOIs) due to the risk of hypertensive crisis.
May interact with CNS depressants, alcohol, and other serotonergic drugs, increasing side effects.
May alter the effects of antihypertensive agents.
Contraindications:
Hypersensitivity to nortriptyline or other tricyclic antidepressants.
Recent myocardial infarction or severe cardiac arrhythmias.
Use with MAOIs or within 14 days of discontinuing them.
Side Effects:
Common: Dry mouth, drowsiness, dizziness, constipation, weight gain, and blurred vision.
Serious: Increased risk of suicidal thoughts in young adults, arrhythmias, and seizures.
Pregnancy & Lactation:
Pregnancy: Use only if clearly needed and prescribed by a physician.
Lactation: May pass into breast milk; consult a physician before use.
Precautions & Warnings:
Use cautiously in patients with a history of seizures, cardiovascular disease, or bipolar disorder.
Monitor for signs of worsening depression or suicidal ideation.
Avoid abrupt discontinuation to prevent withdrawal symptoms.
