Indications:
SERGEL MUPS 40 MG Tablet is indicated for:
Treatment of severe Gastroesophageal Reflux Disease (GERD).
Healing of peptic ulcers caused by H. pylori or NSAID use.
Zollinger-Ellison syndrome.
Erosive esophagitis.
Long-term maintenance therapy for acid-related disorders.
Pharmacology:
Esomeprazole in MUPS form ensures controlled release and optimal absorption. As a proton pump inhibitor (PPI), it inhibits the H+/K+ ATPase enzyme system in gastric parietal cells, reducing gastric acid secretion and facilitating mucosal healing.
Dosage and Administration:
Adults: 40 mg once daily, preferably taken 1 hour before meals.
The treatment duration is usually 4-8 weeks, depending on the condition.
Swallow the tablet whole; do not crush, chew, or split.
Interaction:
May reduce the absorption of drugs like ketoconazole and atazanavir requiring acidic gastric pH.
Use cautiously with anticoagulants such as warfarin due to increased bleeding risks.
May reduce the effectiveness of clopidogrel when administered concurrently.
Contraindications:
Hypersensitivity to esomeprazole, other PPIs, or any components of the formulation.
Severe liver impairment without proper dose adjustments.
Side Effects:
Common: Headache, abdominal discomfort, nausea, diarrhea, and flatulence.
Rare: Hypomagnesemia, vitamin B12 deficiency, and bone fractures with long-term use.
Pregnancy & Lactation:
Pregnancy: Safe under medical supervision (FDA Category B).
Lactation: Use with caution, as esomeprazole may pass into breast milk.
Precautions & Warnings:
Prolonged use may increase the risk of gastrointestinal infections or bone fractures.
Regular monitoring of magnesium and vitamin B12 levels is recommended during extended therapy.
Discontinue immediately if severe allergic reactions occur.
