Indications:
SERGEL MUPS 20 MG Tablet is indicated for:
Gastroesophageal Reflux Disease (GERD).
Peptic ulcers caused by H. pylori infection or NSAID use.
Zollinger-Ellison syndrome.
Erosive esophagitis.
Maintenance therapy for acid-related disorders.
Pharmacology:
Esomeprazole in MUPS form ensures uniform and controlled release in the stomach. It works as a proton pump inhibitor (PPI), inhibiting the H+/K+ ATPase enzyme in gastric parietal cells, effectively reducing stomach acid production and promoting healing.
Dosage and Administration:
Adults: 20 mg once daily, preferably 1 hour before meals.
The duration of treatment depends on the condition, typically 4-8 weeks for GERD.
Swallow the tablet whole with water; do not crush or chew.
Interaction:
May reduce absorption of drugs like ketoconazole and atazanavir requiring an acidic environment.
Use with caution alongside warfarin due to an increased risk of bleeding.
Reduces the efficacy of clopidogrel when used concurrently.
Contraindications:
Hypersensitivity to esomeprazole, other proton pump inhibitors, or any ingredient in the formulation.
Severe liver impairment without appropriate dose adjustments.
Side Effects:
Common: Nausea, headache, diarrhea, flatulence, and abdominal discomfort.
Rare but serious: Hypomagnesemia, vitamin B12 deficiency, and increased fracture risk with prolonged use.
Pregnancy & Lactation:
Pregnancy: Considered safe under medical supervision (FDA Category B).
Lactation: Use with caution as esomeprazole may pass into breast milk.
Precautions & Warnings:
Prolonged use may lead to vitamin B12 deficiency or increase risks of bone fractures.
Monitor magnesium levels during extended treatment.
Discontinue use if severe allergic reactions occur.
