Indications:
HB FIX 20 is indicated for the treatment of:

  • Iron deficiency anemia in patients with chronic kidney disease (CKD)

  • Patients who cannot tolerate or respond inadequately to oral iron therapy

  • Anemia associated with hemodialysis or peritoneal dialysis

  • Postpartum iron deficiency

Pharmacology:
HB FIX 20 contains Iron Sucrose, a water-soluble, complex of polynuclear iron (III)-hydroxide in sucrose. It provides:

  • Rapid replenishment of iron stores

  • Gradual release of bioavailable iron for erythropoiesis

  • Lower risk of free iron toxicity compared to other parenteral iron forms

The iron is utilized in hemoglobin synthesis and stored as ferritin in the liver, spleen, and bone marrow.

Dosage & Administration:

  • Administered only by IV route, either by slow injection or infusion

  • Dose: Typically 100–200 mg elemental iron (5–10 mL) per session, based on patient’s hemoglobin and iron status

  • Maximum single dose: Usually not more than 200 mg

  • Must be diluted in 0.9% sodium chloride if infused

  • Monitor patient for hypersensitivity during and after administration

Interaction:

  • Avoid co-administration with oral iron supplements

  • Iron therapy may interfere with absorption of tetracyclines and other drugs

  • Concurrent use with ACE inhibitors may increase risk of hypotension

Contraindications:

  • Hypersensitivity to iron sucrose or any excipient

  • Anemia not caused by iron deficiency

  • Iron overload or disorders of iron utilization (e.g., hemochromatosis)

Side Effects:

  • Common: Hypotension, nausea, headache, muscle cramps, injection site reactions

  • Rare: Anaphylactic reactions, chest pain, dizziness

  • Post-injection monitoring is essential for allergic responses

Pregnancy & Lactation:

  • Category B: Use only if clearly needed during pregnancy

  • No known risks during lactation, but use under medical supervision

Precautions & Warnings:

  • Monitor serum ferritin and transferrin saturation levels

  • Use caution in patients with liver disease, infection, or inflammatory conditions

  • Avoid repeated or high doses without proper monitoring

  • Resuscitation equipment should be readily available during administration
