Pharmacology:
Isradipine is a dihydropyridine calcium channel blocker that inhibits calcium ion influx into vascular smooth muscle and cardiac muscle cells, causing vasodilation and decreased peripheral vascular resistance. This results in lowered blood pressure and reduced cardiac workload.
Onset: 1–2 hours
Duration: 12–24 hours
Dosage & Administration:
Initial dose: 5 mg once daily
Dose may be adjusted based on blood pressure response, typically 2.5–10 mg daily in divided doses
Take with or without food
Swallow whole with water
Interaction:
May enhance effects of other antihypertensives
Caution with beta-blockers and CYP3A4 inhibitors or inducers
Avoid grapefruit juice as it may increase blood levels
Contraindications:
Known hypersensitivity to Isradipine or other dihydropyridines
Severe hypotension
Cardiogenic shock or unstable angina
Pregnancy and lactation unless clearly necessary
Side Effects:
Common: Headache, dizziness, flushing, peripheral edema
Less common: Palpitations, fatigue, nausea
Rare: Hypotension, tachycardia, allergic reactions
Pregnancy & Lactation:
Pregnancy: Use only if clearly needed; consult physician
Lactation: Safety not well established; consult healthcare provider
Precautions & Warnings:
Monitor blood pressure regularly
Use cautiously in patients with heart failure or liver impairment
Avoid sudden discontinuation
Inform doctor about any adverse symptoms
